By registering for this NACUFS event, you are:
• Giving consent for the use of your name and likeness in event images. The conference sessions and meetings may be recorded and available on demand. Images of the conference participants may be used in conference wrap-up articles and other promotions by NACUFS.
• Giving consent to NACUFS sponsors to contact you via email with limited communications related to their participation at the event.
• Understanding that travel and gathering involves risk of sickness, including sickness from COVID-19. I (and on behalf of my guest(s)) waive and release NACUFS, and their employees and agents, from and against claims, liabilities, and expenses arising from injury, sickness or death from contraction or spread of COVID-19 or other communicable disease due to travel to, or attendance at, an event hosted by NACUFS.
• Agreeing to not attend any NACUFS event if you feel ill or had recent exposure to a COVID-19 case.
• Agreeing this waiver is binding on you and your heirs and successors.
• Agreeing to follow the event safety protocols put in place by NACUFS.
