Applications will be accepted via mail, e-mail, or fax. All applications (including those sent via e-mail ) must include signatures for the applicant and the voting delegate.

APPLICATION PROCESS:

Step 1: Complete the application form, including getting the appropriate signatures.

(Note: Applications missing required signatures risk being omitted from the review process, so please make sure your application is complete and that you and your voting delegate have signed it. If the applicant is the voting delegate, please sign in both places on the application form.)

Step 2: Send the completed application form via mail, e-mail, or fax to:

You will receive an email confirmation when your application is received. If you do NOT receive a confirmation within 48 hours, please contact Colleen Bethea, education assistant, at 517-332-2847 or cbethea@nacufs.org.

Step 3: Applicants selected to attend a particular Institute program will receive a formal invitation from the Lori Mason, director of education, followed by a packet of additional information about the program. That packet will include a date by which you need to confirm your acceptance of the invitation. The registration process is not complete until you return the Participant Information Form in this packet. (Submitting the form after the stated deadline could disqualify you for your seat at the institute.)

CANCELLATION POLICY:

If you are accepted to attend an institute, you will be sent a Participant Information Form, including a date by which you must accept the invitation. Please respond quickly so that the invitation can be extended to someone else if you are no longer able to attend. 

If you have an emergency and need to cancel after that date, please give as much notice as possible, so that one of the designated alternates can be notified and sent instead. (If at all possible, you need to keep your commitment to attend. If you cancel, you are not guaranteed a slot in the next selection process, and must reapply. While true emergencies are certainly taken into consideration, your overall attendance record will be considered when applying for future Institutes.) 

Since the NACUFS Institute programs have limited seats and attendance is determined by a pre-described application/selection process, the decision to cancel after acceptance into a particular program should be reserved for emergency situations only.

You are responsible for cancelling your own hotel and travel arrangements.

NACUFS reserves the right to cancel this program at any time.
